Introduction
Applicants may wish to propose a partnership-based delivery model for the OPHRS. Under this arrangement, DGS would enter into a funding agreement with a single lead agency. The lead agency would be responsible for coordinating service delivery, managing and distributing funding to partner agencies, and meeting all reporting and compliance requirements. Partner agencies would deliver services through formal partnership arrangements with the lead agency and would not have a direct contractual relationship with the Government. If applying with this model, applications should be made by the intended lead agency, and delivery partners listed in Section 1D.

What you will need to complete this application

Please ensure you have the following information available before starting:

  • Australian Business Number (ABN) and organisational contact details

  • The organisation’s bank account details

  • The name, ABN and legal status of any partner agencies if applicable

  • Responses to the assessment criteria published in the Request for Service guidelines

  • Details for two referees, including their contact information

  • Authorisation to submit this application on behalf of the Chief Executive Officer (CEO)
